These forms are included in the Athlete's Packet.

|
Liability Waiver Forms:
A parent or guardian must sign the appropriate Liability Waiver Form and forward it to your State Selection Chair as soon as possible.
|
|
You may confirm your Invitation acceptance by phone, Fax, or E-mail, however, the Liability Waiver form must be signed by a parent or guardian and forwarded to your State Selection Chair in order to compete, and you are required to bring a signed copy to Meet Check-in.
Athletes who show up for the meet without a
signature will NOT be allowed to participate.

Travel Information Form
All travelers are encouraged to make arrangements with the Transportation Committee by submitting their information to the Committee contact listed on the form. You may submit the form by U.S. Mail, or E-mail. ...More

No Stay, No Compete Form
The MCM Elegante Hotel is the exclusive designated headquarters for the 36th Annual Great Southwest Track & Field Classic. Athlete's must complete the "No Stay, No Compete" Form in order to participate in the meet.
